Bone Injuries Associated with Acute Anterior Cruciate Ligament Disruption



Abstract
Bone injury associated with anterior cruciate ligament (ACL) rupture is considered an important prognostic factor in that it can cause persistent symptom and/or degenerative changes of articular surface .
We analyzed bony lesions on the MR image in 33 patients whose ACL rupture were ascertained by arthroscopic examination. Bony lesion occurred in all of 12 patients (100%)with complete ACL tear. Nine patients(69%)among 13 patients with major partial tear involving more than 1/2 of the ligament had bony lesions. Bony injuries were found in 4 patients(50%)among 8 patients with minor partial tear involving less than 1/2 of the ligament. MR signal abnormalities were mostly located in the middle to anterior femoral condyle and posterolateral tibial plateau. ACL disruption associated with of other ligaments in the knee had more frequent bony lesion than isolated ACL injury. Lateral meniscus tear accompanying ACL injury had same compartment bony lesion more frequently than medial meniscus tear.
Concerns for bony lesion associated with ACL disruption appeared to be important in clinical setting.
